Приведены результаты лечения 68 пациентов, которым было отказано в проведении стандартного хирургического лечения из-за преклонного возраста или тяжелых сопутствующих заболеваний. У 63 больных было диагностировано единичное новообразование, у 5 – по два опухолевых очага. Для проведения фотодинамической терапии использовали четыре отечественных препарата: фотогем, фотосенс, радахлорин и аласенс. Сеанс облучения выполняли под местной анестезией во время стандартного эндоскопического исследования, при этом использовались диодные лазеры с длиной волны, соответствующей полосе поглощения фотосенсибилизаторов (фотогем – 630 нм, фотосенс – 670 нм, аласенс-индуцированный протопорфирин IX – 635 нм, радахлорин – 662 нм). Результат оценивался через 1 мес. после проведения лечения на основании данных эндоскопического, морфологического исследований, КТ, УЗИ, эндосонографии. В результате лечения 73 опухолевых очагов достигнута полная регрессия 53 (72,6%) и частичная регрессия 20 новообразований (27,4%). Показано, что эффективность фотодинамической терапии зависит от размера опухолевого поражения. Так, при размере опухолей до 1 см полная регрессия отмечена в 100% случаев, до 1,5 см – в 70,8%, до 3 см – в 65,2%, до 5 см – в 58,3%. Медиана выживаемости больных составила 7,31 лет, 3-летняя – 83±5%, 5-летняя – 69±8%. Накопленный опыт показывает, что разработанные варианты фотодинамической терапии перспективны для лечения начальной формы рака желудка в качестве альтернативы хирургическому лечению.</p><p> </p></abstract><trans-abstract xml:lang="en"><p>The experience of photodynamic therapy for early gastric cancer is described in the article. The treatment results in 68 patients who were excluded for convenient surgical treatment because of advanced age or severe co-morbidity are represented. 63 patients had single tumor, 5 patients – 2 tumors. Four Russian agents: photogem, photosens, radaсhlorin and alasens, were used for photodynamic therapy. The treatment session was performed under local anesthesia during routine endoscopy with diode laser with wavelength consistent with photosensitizer (photogem – 630 nm, photosens – 670 nm, alasens-induced protoporphyrin IX – 635 nm, radaсhlorin – 662 nm). The short-term results were analyzed 1 month after treatment according to endoscopy, morphological study, CT, ultrasound or endosonography. For 73 lesions complete regression was observed in 53 (72.6%) and partial regression in 20 tumors (27.4%). The efficacy of photodynamic therapy was shown to be directly associated with tumor size. Thus, for tumors up to 1 cm regression occurred in 100% of cases, up to 1.5 cm – in 70.8%, up to 3 cm – in 65.2%, up to 5 cm – in 58.3%. The median survival rates accounted for 7.31 years, 3-year survival – 83±5%, 5-year - 69±8%.
